Four Steps to Get Started

2. Choose What Works for You
We can schedule a 15-minute consultation to answer your questions, or you can schedule your first appointment with Dr. Salazar.

3. Access Your Client Portal
Once your appointment is scheduled, whether in person or online, you’ll receive an invitation to our secure client portal.

4. Complete Your Forms
Before your first appointment, please complete the required forms in the client portal. This provides helpful background and allows us to focus more fully on you during our time together.
